Heart Failure Assessment Tool on My Health E-Vet website
CHF QUERI has collaborated with the My Health E-Vet (MHV) in providing a Heart Failure Assessment Tool for self-management by heart failure patients. This tool will be helpful to these patients to understand and track their health condition. Patients can use this tool to determine if their heart failure care is consistent with current guideline recommendations.
