- Materials Developed by VA Facilities & Selected by CHF QUERI: A committee of VA experts including clinicians, nurses and patient educators have reviewed these heart failure related patient education materials and we have obtained permission form persons who developed these materials to post them on this website. At no cost these materials are available for view, download, print or copy. When the cursor is placed on the title of the material details will show up in terms of media type, format, number of pages, B&W/color, intended audience, source and Flesch-Kincaid Grade Level for which scoring maybe affected by the pharmaceutical names and names of certain health conditions in the material.
- Materials Developed by the VA for MOVE Program:
is a national weight management program designed by the VA National Center for Health Promotion and Disease Prevention (NCP), a part of the Office of Patient Care Services, to help veterans lose weight, keep it off and improve their health. These patient education materials are in PDF format and maybe printed in B&W or color. These materials are available at no cost by clicking http://www.move.va.gov/
- Materials Developed by Non-VA Organizations: Excellent patient education materials related to heart failure have been developed by the American Heart Association, Heart Failure Society of America, Health Informatics International and Krames on Demand. We have provided brief description and links to these websites. These materials may be obtained at a cost or free of charge.
